Academic Record Update Forms

Forms	Purpose
ICS Prerequisite Clearing Request *Use the UCI VPN to access the Form if off-campus	Course prerequisite(s) were satisfied outside of UCI. For example, courses taken through community college, other 4 year institutions, or DCE. Estimated Processing: 3-4 business days
ICS Degreeworks Update Form	ICS degreeworks update form can be used for: Updating GE and ICS major requirements Adding/dropping minors and updating minor requirements Applying transfer credit or exam score credit to degree requirements Estimated Processing: 2 - 3 weeks
Declare or Update Specialization/Track	ICS majors that require a specialization or track are: Computer Science, Computer Game Science, and Informatics To declare or update your specialization/track for your major complete the form Estimated Processing: 2 - 3 weeks
Academic Plan Review Form	This form is only for declared ICS Majors to have their academic plans reviewed. Estimated Processing: 2-4 weeks

Petitions

Petition	Purpose/ Examples
General Undergraduate Petition	Submit this form to request an exception to Policy: Course Substitution Time Conflict Request more than 20 units Other Exceptions to Policy Note: Supporting documentation may be required. Please review our ICS Undergraduate Student Policies page prior to submitting this form. Estimated Processing: 2 - 3 weeks
Syllabus Review for Course Equivalency	Students MUST submit the pre-check questionnaire before they can petition for transfer credit. If you are eligible for a syllabus review, the form will be provided once all criteria are met. Syllabi reviews are only accepted Monday of Finals Week at 8am through Friday of Week 2 at 5pm for each quarter. Request credit for a course taken at a different institution. For CA community colleges, students must complete the entire series as listed on assist.org at one community college to earn credit. We do not accept mixing and matching coursework between different institutions or award partial credit for incomplete series. We will only review exceptions for incoming students during summer orientation and Fall quarter. Estimated Processing: 3 - 4 weeks
Request Study Abroad Syllabus Review Review Study Abroad Policies	For students who have already been accepted in a study abroad program or have returned from their study abroad program Request credit for courses taken at an institution abroad Estimated Processing: 3 - 4 weeks

The “Dean’s Signature”

The Dean’s Signature may be obtained at the ICS Undergraduate Student Affairs Office.

Counselors are authorized to provide Dean’s review and signature in all instances involving LATE add/drop/change of grading option requests done via the on-line Enrollment Exceptions system, and on other student-related forms issued by the Registrar’s Office or other administrative offices.
