Fill in the form
Select UPDATE if the form is not yet complete and you wish to save the FCAR form for later
Select FINAL if the form is complete and you wish to obtain the FCAR document
Press the "THIS BUTTON" button to update or finalize the FCAR
Save the resulting FCAR form or document using your browser File button (save as a single html file)
(if using the Chrome browser then save as webpage complete)
Email the saved html document (as an attachment) to dhirschb [at] ics.uci.edu

Note: The form will retain updated values when saved and can be subsequently modified.
The document cannot be modified within your browser.
The document is an html file that can be sent via email as an attachment.
